Your Accessible Care is a registered NDIS provider delivering person-centred supports shaped around each participant's goals, preferences and everyday life. Support that starts with the person

We begin by listening. Together, we identify what matters to you, understand the support you need and develop an approach that respects your choices, routines, culture and goals. As needs change, we review and adapt the support with you.

How we may support you

Service availability depends on location, assessed needs, participant goals, funding arrangements and YAC's current capacity. We will confirm fit before services commence.

  • Daily living and personal support

    Assistance with everyday routines and personal activities, delivered with dignity and respect.

  • Community participation

    Support to access community, social, recreational and capacity-building activities.

  • Transport and lifestyle support

    Support for agreed appointments, activities and community participation where this is within service scope.

  • Capacity building

    Practical support to build confidence, skills and greater independence.

If your travel enquiry includes disability-related support needs, YAT can ask for your consent to introduce you to YAC. Choosing to speak with YAC does not create a service agreement, and funding is never guaranteed.

Your Accessible Travel designs and coordinates accessible travel experiences. Where eligible disability-related supports are required, these supports are assessed, contracted, delivered and invoiced separately by Your Accessible Care, a registered NDIS provider.

How it works

From first conversation to agreed support

    Tell us what matters

    Share your goals, preferences, communication needs and the support you are seeking.

    Confirm service fit

    YAC discusses needs, location, capacity, risks and funding arrangements.

  3. 3

    Agree the support

    If YAC can safely meet the need, the participant and YAC agree a service agreement and support plan before delivery.

  4. 4

    Deliver, review and adapt

    Support is delivered, documented and reviewed with the participant.

If travel is involved, a separate Your Accessible Travel agreement, quote and invoice may also apply. Travel costs and disability support costs are always explained and billed separately.

Participants and families

Clear information before you decide

  • You remain at the centre of decisions about your support.
  • You can involve a family member, nominee, advocate or support coordinator.
  • Support costs and travel costs are explained separately.
  • We discuss communication, accessibility, cultural and safety needs before service starts.
  • You can give feedback or make a complaint without it affecting your right to respectful support.

Support coordinators

A clearer pathway for referrals

We aim to make service-fit discussions practical and transparent. Please share the participant's goals, requested support, preferred timing, location, communication needs, risk considerations and plan-management details. We will confirm whether the enquiry is within YAC's verified scope and current capacity.

Rights and quality

Support underpinned by rights, safety and accountability

  • Participant rights and choice

    Support is planned with the participant, with clear information about options, and the right to change or stop support.

  • Worker screening and training

    Workers are engaged through screening, onboarding, training and supervision appropriate to the agreed support.

  • Risk and incident management

    Risks are discussed before service starts, documented, and incidents are recorded, reviewed and acted on.

  • Feedback and complaints

    Feedback can be raised at any time, in the way that suits you, and can be made by a nominee or advocate.

  • Privacy and information handling

    We collect only the information needed, explain why it is needed, and use secure processes for sensitive documents.

  • Continuous improvement

    What we learn from reviews, feedback and incidents is used to improve how support is planned and delivered.
