For professionals

A referral pathway with clear scope and documentation.

For support coordinators, plan managers and allied health professionals: what we do, what we do not do, and how a referral is handled.

Scope and boundaries

What we do

  • Person-led planning for travel and meaningful experiences
  • Supplier and destination enquiry using specific, verifiable questions
  • Written proposals with inclusions, exclusions and unconfirmed items
  • Separated travel and disability support cost categories
  • Coordination with the participant's existing support network, with consent

What we do not do

  • Provide funding advice or confirm plan eligibility
  • Present travel costs as disability supports
  • Offer clinical, legal or emergency services
  • Publish access features or NDIS details that have not been verified
  • Take online bookings or payments

What to expect after a referral

  1. Step 1

    Acknowledgement

We confirm receipt of your referral and tell you who is reviewing it.

  2. Step 2

    Initial review

We consider the participant's goals, the experience type and whether an eligible support pathway may be relevant.

  3. Step 3

    Clarifying conversation

We talk with the participant, and with you where consent allows, to understand preferences and practicalities.

  4. Step 4

    Written outline

You receive a written outline of what is planned, what is excluded, what is unconfirmed, and how costs are split.

  5. Step 5

    Decision gate

Nothing proceeds until agreements, suitability and arrangements are confirmed in writing.

Travel costs and disability support costs

Travel costs - including flights, accommodation, meals, activities and general transport - are separate from disability support costs. Some disability support costs may be claimable where permitted under current NDIS rules and the participant's plan. Eligibility must be confirmed individually.

Travel and experience costs

Generally contracted and invoiced by Your Accessible Travel. Includes flights, accommodation, meals, activities and general transport.

Eligible disability support costs

Separately assessed, contracted and invoiced by Your Accessible Care where applicable, subject to individual assessment and operational availability.

This website does not provide personal funding advice. Eligibility must be confirmed individually.

Make a referral

Please do not attach or paste sensitive health, identity or plan documents. Share only what is needed for an initial conversation, with participant consent.

Initials or a first name are enough at referral stage.

Please do not upload or paste health records, plan documents or unnecessary personal information.

Have a question before referring?

We are happy to talk through scope, timelines and how costs would be presented to the participant.
