Future support pathway through YAC
Support pathway through Your Accessible Care
Where eligible disability-related supports are required for an experience, Your Accessible Care - a registered NDIS provider - assesses, contracts, delivers and invoices those supports separately. This pathway is being established.

Who it may suit
This is a starting point for a conversation. Suitability is always assessed individually with the person, and we will say honestly if something is not a good fit.
- People who may need eligible disability-related supports to take part in an experience
Location concept: Assessed individually, subject to scope and operational availability
Proposed highlights
Concept only - nothing here is confirmed.
- Separate service agreement with Your Accessible Care
- Individual assessment before any support is confirmed
- Clear separation between travel arrangements and support arrangements
Access and communication considerations
Planning considers physical, sensory, cognitive, communication, psychosocial and multiple access requirements together. We plan around the person, not a diagnosis.
- Individual assessment of support needs, risks and preferences
- Communication and consent arrangements
- Continuity with existing supports where a person already has them
- Records, incidents and complaints handled under provider processes
What may be included, excluded and still to be confirmed
May include
- Assessment conversation and, where appropriate, a separate service agreement
Not included
- Any guarantee that a support worker or service is available
- Any confirmation of individual funding eligibility
Still to be confirmed
- Operational availability, which is being established
- Individual eligibility, which must be confirmed personally
Costs are always kept separate
Travel costs
- Travel costs remain contracted and invoiced by Your Accessible Travel
Disability support costs
- Eligible disability support costs are separately assessed, contracted and invoiced by Your Accessible Care
Travel costs and disability support costs
Travel costs - including flights, accommodation, meals, activities and general transport - are separate from disability support costs. Some disability support costs may be claimable where permitted under current NDIS rules and the participant's plan. Eligibility must be confirmed individually.
Travel and experience costs
Generally contracted and invoiced by Your Accessible Travel. Includes flights, accommodation, meals, activities and general transport.
Eligible disability support costs
Separately assessed, contracted and invoiced by Your Accessible Care where applicable, subject to individual assessment and operational availability.
This website does not provide personal funding advice. Eligibility must be confirmed individually.
